Creating Forms that insert the Page Name (or other fields) into the Subject field Print

When creating a new form or editing an existing form, you can have the Page/Post Title automatically inserted into the email subject line.

For an existing form:

Edit the form
Select: Notifications
On the Subject Line Enter: New submission from {form_title} -{embed_post:post_title}

This will embed the post/page title into the subject line.

Additionally you can insert other information in the subject line by using the

“Insert Merge Tag” drop down box. This same feature is available to customize the content in the email you receive

You can also view all the forms submitted from your site by looking at Form Entries in the Forms sidebar. Here you can view the details of each form request.
